Important Medical and Billing Disclosures
Last updated April 10, 2026. These disclosures summarize the most important clinical, eligibility, billing, and state availability limits that apply to Motive services.
1. Motive is not a medical provider
Motive supports digital intake, billing review, provider-reviewed next-step coordination, and non-clinical operations. Clinical decisions remain with licensed providers and clinical entities. Nothing on this site replaces the advice, diagnosis, or treatment of your personal medical professionals.
2. No guarantee of eligibility, treatment, or prescription
Completing an assessment, joining a membership, or paying a membership fee does not guarantee provider approval, treatment eligibility, or receipt of a prescription. A licensed provider may determine that no treatment is appropriate, or that additional information, testing, or a live visit is required first.
3. Telehealth and state availability
Services may not be available in all states or territories. State law may require an audio or video visit, an in-person visit, identity verification, additional forms, lab testing, or other steps before treatment can be considered. Availability and clinical workflows may change without notice based on licensing or regulatory requirements.
4. GLP-1 and weight-treatment risk summary
Weight-related treatments, including GLP-1-based therapies when prescribed, may involve serious risks, side effects, contraindications, and monitoring requirements. Potential issues can include gastrointestinal side effects, dehydration, gallbladder complications, pancreatitis, kidney-related complications, thyroid-related concerns, allergic reactions, and interactions with other conditions or medications.
Treatment is not appropriate for everyone. You should disclose all relevant medical history, current medications, allergies, pregnancy or breastfeeding status, and prior adverse reactions so a provider can make an informed decision.
5. Billing disclosure
Unless a different offer is shown at checkout, the current Motive introductory first month of membership is $29 charged today. After the introductory period, membership automatically renews at $99/month until canceled before the next renewal date.
Membership generally covers platform access, billing flow, and provider-reviewed next-step coordination. If treatment options are appropriate after provider review, pricing for medication, pharmacy fulfillment, laboratory services, or other treatment-related costs will be shared before you choose whether to continue into that part of care, unless a selected plan expressly states otherwise.
By completing checkout, you authorize recurring charges in line with the terms presented at checkout and in Terms of Service. Cancel future renewals before the next billing date to avoid the next membership charge.
6. Insurance and reimbursement
Insurance coverage is not guaranteed. Coverage varies by plan, state, diagnosis, pharmacy benefit design, provider determination, and treatment pathway. You are responsible for confirming your own coverage and any out-of-pocket obligations.
7. Member responsibilities and follow-up
You are responsible for providing accurate and current information, responding to requests for follow-up, completing any required monitoring, and informing your provider about new diagnoses, medication changes, side effects, or other clinically relevant updates.
8. Not for emergencies
Motive is not intended for emergencies. For urgent symptoms or emergencies, call 911 or seek immediate care.
