Terms of Service
Last updated April 10, 2026. These Terms of Service govern your use of the Motive website, digital intake flows, billing flow, and any non-clinical platform tools made available through https://trymotive.co.
1. Acceptance of these terms
By accessing or using Motive, you agree to these Terms of Service and our Privacy Policy. If you do not agree, do not use the site or submit health information through the assessment experience.
You must be at least 18 years old and located in the United States to use the service. Services may not be available in every state, and availability may change based on licensing, telehealth, or operational requirements.
2. What Motive does and does not do
Motive operates as a digital intake, access, and coordination platform. Motive itself does not practice medicine, prescribe medication, operate a pharmacy, or replace the judgment of a licensed medical professional.
Any clinical review, eligibility determination, diagnosis, treatment recommendation, or prescription decision is made solely by a licensed provider or another appropriately credentialed clinical entity. Use of this site does not guarantee eligibility, treatment, or a prescription.
3. No emergency or crisis use
Motive is not an emergency service. Do not use this site for urgent or life-threatening symptoms. If you believe you are having a medical emergency, call 911 or go to the nearest emergency room immediately.
4. Assessment, provider review, and treatment decisions
The online assessment is informational and intake-oriented. It helps collect information that may be used to determine whether a provider review is appropriate. Assessment results, plan previews, or care pathway language are preliminary and do not constitute a diagnosis, treatment plan, or medical advice.
A licensed provider may require additional information, lab work, an audio or video visit, or other follow-up before making a clinical decision. A provider may also determine that treatment is not clinically appropriate for you.
5. Membership, billing, cancellation, and refunds
Unless a different offer is presented at checkout, Motive currently offers an introductory first month of membership for $29 charged at signup. After the introductory period, membership automatically renews at $99/month until canceled before the next renewal date.
Membership fees cover platform access, administrative services, billing flow, and provider-reviewed next-step coordination. If treatment options are appropriate after provider review, pricing for medication, pharmacy fulfillment, lab work, clinician-directed treatment costs, and other third-party charges will be shared before you choose whether to continue into that part of care, unless expressly included in a selected plan.
By completing checkout, you authorize recurring charges in accordance with the pricing disclosed at checkout. You may cancel future renewals through your member account or by contacting [email protected] before your renewal date. Unless otherwise stated during checkout or required by law, charges are generally non-refundable once services have begun.
Motive may update future pricing, promotions, or membership features on a prospective basis. Any change will apply only after notice and will not retroactively modify charges already incurred.
6. Your responsibilities
You agree to provide accurate, complete, and current information, including health and billing details.
You are responsible for reviewing provider communications, following instructions about follow-up care, and notifying your provider about changes to your health, medications, allergies, pregnancy status, or other clinically relevant facts.
You may not use the site in a fraudulent manner, impersonate another person, interfere with platform security, scrape the service without authorization, or use the service in violation of applicable law.
7. Third-party services
Motive may rely on third-party vendors and clinical partners, including payment processors, analytics providers, pharmacies, laboratories, and telehealth or care-delivery organizations. Those third parties may maintain their own terms, privacy notices, or professional standards.
8. Intellectual property
The Motive website, branding, copy, design system, software, and related materials are owned by Motive Health, Inc. or its licensors and are protected by applicable intellectual property laws. Except for the limited right to use the site for personal, lawful purposes, no other rights are granted.
9. Disclaimers and liability limits
The site and non-clinical services are provided on an "as is" and "as available" basis. To the fullest extent permitted by law, Motive disclaims warranties of merchantability, fitness for a particular purpose, non-infringement, and uninterrupted availability.
To the fullest extent permitted by law, Motive and its administrative affiliates will not be liable for any indirect, incidental, special, consequential, punitive, or exemplary damages arising from your use of the site or membership services. Nothing in these terms limits liability that cannot be limited under applicable law.
10. Changes to the service or these terms
Motive may update, suspend, or discontinue parts of the service at any time. We may also revise these Terms of Service from time to time. The updated version becomes effective when posted on this page, unless a later effective date is stated.
