Privacy Policy

Last updated April 10, 2026. This Privacy Policy explains how Motive Health, Inc. collects, uses, discloses, and protects information when you use Motive online services, submit an assessment, or interact with our intake and billing experience.

1. Information we collect

We may collect the following categories of information:

  • Identifiers and contact details such as your name, email address, phone number, and mailing details.
  • Assessment and health-related information you choose to provide, such as age, height, weight, medical history, goals, medications, allergies, and related intake responses.
  • Billing and transaction information, including payment status and subscription history. Payment card data is typically processed by our third-party payment providers rather than stored directly by Motive.
  • Device, browser, analytics, and usage information, including approximate location, pages visited, referral information, and interactions with site content.
  • Communications you send to support, billing, or coordination contacts.

2. How we use information

We use information for the following business, billing, and coordination purposes:

  • Operate the website, assessment flow, billing flow, and account experience.
  • Coordinate provider review, pharmacy fulfillment, or other care-related next steps when appropriate.
  • Process payments, manage subscriptions, and maintain billing records.
  • Respond to support inquiries and communicate about your account or requested services.
  • Measure site performance, improve usability, monitor security, and prevent fraud or abuse.
  • Comply with legal obligations and enforce our terms, policies, and business records requirements.

3. How we disclose information

We may disclose information to the following categories of recipients when reasonably necessary:

  • Licensed providers, clinical groups, and care-delivery partners involved in your review or treatment.
  • Pharmacies, laboratories, and treatment-fulfillment partners when required for care coordination.
  • Payment processors, hosting providers, analytics vendors, and other service providers acting on our behalf.
  • Professional advisors, auditors, insurers, or regulators where needed for compliance or business operations.
  • Law enforcement or other parties when disclosure is required by law, court order, or to protect rights and safety.
  • A successor entity in connection with a merger, financing, sale of assets, or similar corporate transaction, subject to applicable confidentiality obligations.

4. Health information and provider privacy notices

Some information you submit may be shared with a licensed provider or clinical partner for evaluation. Those providers or entities may be subject to their own legal and professional confidentiality obligations, including a separate Notice of Privacy Practices where applicable.

When a provider or clinical partner maintains your medical record as a covered entity or health care provider, that party's privacy notice may govern certain uses and disclosures of protected health information in addition to this Privacy Policy.

5. Cookies, analytics, and similar technologies

Motive uses cookies and similar technologies to keep the site functioning, remember preferences, understand site traffic, and measure conversion performance. We currently use analytics tools such as Google Analytics and Microsoft Clarity to understand traffic patterns and improve site experience.

You can limit cookies through your browser settings, but parts of the site may not function properly if you block essential technologies.

6. Data retention and security

We retain information for as long as reasonably necessary to operate the service, fulfill coordination and compliance obligations, resolve disputes, enforce agreements, and maintain business records.

We use administrative, technical, and organizational safeguards designed to protect information. No system can guarantee absolute security, and you use the service with that understanding.

7. Your choices and privacy requests

You may request access to or correction of certain account information by contacting [email protected].

You may also opt out of non-essential marketing communications using the unsubscribe instructions in those messages. Transactional, account, billing, and care-coordination communications may still be sent as needed to operate the service.

Residents of certain states may have additional privacy rights under applicable law. We will evaluate and respond to verified requests in accordance with applicable requirements.

8. Children's privacy

Motive is intended for adults and is not directed to children under 18. We do not knowingly collect personal information directly from children through this site.

9. Changes to this policy

We may update this Privacy Policy from time to time. The updated version will be posted here with a revised effective date. Material changes will apply prospectively unless otherwise required by law.
